Eligibility

• Open to legal residents of Ohio who are 18 years or older.
• Employees of Five Star Home Services, its affiliates, subsidiaries, advertising or promotional agencies, and the immediate family members or household members of such employees are not eligible to enter or be nominated.
• Nominees must own the home where the furnace will be installed. Proof of home ownership may be required upon request.
• The home must be located within Five Star Home Services’ serviceable zip code area.

How to Enter

1. Visit the Five Star Home Services website and complete the official nomination form.
2. Each nomination must include the nominee’s name, contact information, and a brief story explaining why the nominee should be considered.

• Limit one (1) nomination per person.
• All nominations must be submitted by 11:59 PM EST on November 30, 2025.

Winner Selection

• One (1) recipient will be selected on or about December 3, 2025 by a review panel appointed by Five Star Home Services, based on need, story impact, and eligibility.
• Odds of selection depend on the number of eligible nominations received.

Prize Details

One (1) prize will be awarded. The prize consists of a brand-new Five Star furnace (80% Single Stage or 96% Single Stage), including delivery and standard installation.

Approximate Retail Value (ARV): up to $7,167.

Installation includes:

• Removal of existing furnace equipment
• Reconnection to existing ductwork, electrical, and condensate drain systems
• Permits (if required by municipality)
• Full system testing and start-up
• Professional cleanup of the work area

Warranty

• Includes a 10-Year Parts Warranty and 10-Year Labor Warranty.
• Annual maintenance required by manufacturer to maintain parts warranty.
• Annual maintenance through Five Star required to maintain labor warranty.
• Failure to perform maintenance may void warranties.

Limit: One (1) furnace per home. In-home consultation required prior to installation.

Notification

• The selected recipient will be notified via email or phone no later than December 5, 2025.
• If the selected recipient cannot be contacted or fails to respond within 72 hours, an alternate recipient may be chosen.

Publicity Release

By participating, nominators and nominees agree that Five Star Home Services may use their name, likeness, photographs, videos, nomination story, and other submitted content for marketing, advertising, and promotional purposes, including on social media and local media outlets, without additional compensation or approval, unless prohibited by law.

Tax Responsibility

The recipient is solely responsible for all applicable federal, state, and local taxes associated with the prize.

Because the approximate retail value of the prize exceeds $600, the recipient will be required to complete a Form W-9 and will receive an IRS Form 1099 for the retail value of the prize, which will be reported to the Internal Revenue Service.

Release of Liability

By participating, all entrants agree to release and hold harmless Five Star Home Services, its affiliates, subsidiaries, advertising and promotion partners, and their respective officers, directors, employees, and agents from any and all liability, loss, or damage arising out of participation or prize acceptance, installation, or use.

Five Star Home Services is not responsible for pre-existing illegal conditions or structural deficiencies within the home.

Five Star Home Services shall not be liable for any damages due to the inability of the structure to support equipment or for the identification, handling, or removal of hazardous materials (including asbestos, certain refrigerants, or oils). If such materials are discovered, work may be suspended until hazards are mitigated by a licensed contractor.

Disputes

All disputes shall be governed by and construed under the laws of the State of Ohio, without regard to conflict of law provisions. All actions shall be filed in the state or federal courts located in Ohio, and participants consent to the jurisdiction of such courts.
